(Mayo Clinic) Psychological Factors Some of the psychological factors or emotional health include people with a negative body image. They may have low self-esteem, perfectionism impulsive behavior and troubled relationships with friends and loved ones. (Article 4) Environmental Factors Environmental factors or social factors that would contribute to the occurrence of eating disorders are dysfunctional family dynamic, professions and careers that promote being thin and weight loss, such modeling. People that suffer from anorexia use dangerous efforts to control their weight and shape, which often knowingly interferes with their lifestyle. Obsession with calories and fat contents of food or use other methods to lose weight; such as having engaging in formalized eating patterns, including, cutting food into tiny pieces, eating alone, and/or hiding food. (Cite article 5) Bulimia Nervosa Bulimia is a serious, theoretically life-threatening eating disorder. When you have bulimia, you have episodes of bingeing and purging that involve feeling a lack of control over your eating. Many people with bulimia also restrict their eating during the day, which often leads to more binge eating and purging (Cite article 3). During these episodes, bulimics typically eat a large amount of food in a short time, and then try to rid of the extra calories in an unhealthy way. Because of guilt, shame and an intense fear of weight gain from overeating, they may force vomiting (purging bulimia), exercise too much, or use other methods, such as laxatives, to get rid of the calories (no purging bulimia). Even if you are bulimic, you’re undoubtedly preoccupied with weight and body shape and may judge severely and harshly for self-perceived flaws. Bulimic may be at a normal weight or even a bit overweight. (Cite article 4) Binge-Eating Disorder When it comes to a binge-eating disorder, many regularly eat too much food (binge) and feel a lack of control over eating. Eating quickly or eat more food than intended, even when not hungry, and may continue eating even long after becoming uncomfortably full. After a binge, there may feeling of guilty, disgusted or ashamed by the behavior displayed and the amount of food eaten (Cite Article 2). Many will try to but do not try to compensate for this behavior with excessive exercise or purging, as someone with bulimia or anorexia might. Embarrassment can lead to eating alone to hide bingeing. A new round of bingeing usually occurs at least once a week. The team typically includes medical providers, mental health providers and dietitians; all with experience in eating disorders. However, in general, it typically includes psychotherapy, nutrition education and medication thorough being either an outpatient, or inpatient and if your life is at risk the possibility of being hospitalized. Psychotherapy can include both Cognitive Behavioral therapy and Family Based Therapy. (Cite Article 3) Cognitive Behavioral Therapy Cognitive Behavioral Therapy (CBT) is applicable to all eating disorders but has been most intensively studied in the treatment of bulimia nervosa (BN). CBT is designed to alter abnormal attitudes about body shape and weight. (CITE ARTICLE 1) CBT is effective in reducing all core features of BN and shows good maintenance of therapeutic improvement. Family Based Therapy Family Based Therapy (FBT) is an evidence-based treatment for children and over teenagers dealing with eating disorders. The family is involved in making sure that the child or other family member follow healthy-eating patterns and maintains a healthy body image and weight. (Cite article 1) Outpatient: The goals of this form of treatment include restoring a normal weight and maintaining that weight, managing physiologic abnormalities, reducing the comorbid psychiatric symptoms such as depression, and changing the fundamental thoughts that are underlying the anorexia. Bulimics require similar medical management with the goal of the treatment being the restoration of a normalized eating pattern free of the binge and/or purge cycle. The partial hospitalization treatment should follow a group treatment model and should include nutritional counseling, meal planning and additional groups such as social skills training. For a patient with BN who are in transition from inpatient to outpatient treatment or those whose bingeing and purging have begun to interfere with their functioning may be treated in partial hospitalization. (Article 3) Inpatient Treatment: The criteria need for hospitalization for both anorexia and bulimia include unstable medical conditions. Weight loss of 10% to 15% or more normal for relapse only or 16% to 20% or more if it is a patients’ first episode. (Cite article 1). The inpatient hospitalization beings with a full evaluation, including a psychiatric and medical evaluation. Patients with anorexia are fed initially with supplemental liquid feedings 6 times a day until they are within at least 10% of their goal weight. Despite being born into one of the â€Å"400 Families,† the richest and most influential families in New York, Eleanor Roosevelt’s childhood was not a happy one. Eleanor’s mother Anna was considered a great beauty while Eleanor herself was not, a fact that Eleanor knew greatly disappointed her mother. On the other hand, Eleanor’s father Elliott doted on her and called her â€Å"Little Nell,† after the character in Charles Dickens’ The Old Curiosity Shop. Unfortunately, Elliott suffered from a growing addiction to alcohol and drugs, which ultimately destroyed his family. In 1890 when Eleanor was about 6 years old, Elliott separated from his family and began receiving treatments in Europe for his alcoholism. At the behest of his brother Theodore Roosevelt (who later became the 26th president of the United States), Elliott was exiled from his family until he could free himself from his addictions. Anna, missing her husband, did her best to take care of Eleanor and her two younger sons, Elliott Jr., and baby Hall. Then tragedy struck. In 1892, Anna went to the hospital for a surgery and afterward contracted diphtheria; she died soon after when Eleanor was 8 years old. Just months later, Eleanor’s two brothers came down with scarlet fever. Baby Hall survived, but 4-year-old Elliott Jr. developed diphtheria and died in 1893. With the deaths of her mother and young brother, Eleanor hoped she would be able to spend more time with her beloved father. Not so. Elliott’s dependency on drugs and alcohol got worse after the deaths of his wife and child, and in 1894 he died. Within 18 months, Eleanor had lost her mother, brother, and father. She was a 10-year-old orphan. Eleanor and her brother Hall went to live with their very strict maternal grandmother Mary Hall in Manhattan. Eleanor spent several miserable years with her grandmother until she was sent abroad in September 1899 to Allenswood School in London. Education Allenswood, a finishing school for girls, provided the environment 15-year-old Eleanor Roosevelt needed to blossom. While she was always disappointed by her own looks, she had a quick mind and was soon picked as a â€Å"favorite† of the headmistress, Marie Souvestre. Although most girls spent four years at Allenswood, Eleanor was called home to New York after her third year for her â€Å"society debut,† which all wealthy young women were expected to make at age 18. Unlike her wealthy peers, however, Eleanor did not look forward to leaving her beloved school for an endless round of parties she found meaningless. Meeting Franklin Roosevelt Despite her misgivings, Eleanor returned to New York for her society debut. The entire process proved tedious and bothersome and made her once again feel self-conscious about her looks. There was, however, a bright side at her coming home from Allenswood. While riding on a train, she had a chance encounter in 1902 with Franklin Delano Roosevelt. Franklin was a fifth cousin once removed of Eleanor’s and the only child of James Roosevelt and Sara Delano Roosevelt. Franklin’s mother doted on him- a fact that would later cause strife in Franklin and Eleanor’s marriage. Franklin and Eleanor saw each other frequently at parties and social engagements. Then, in 1903, Franklin asked Eleanor to marry him and she accepted. However, when Sara Roosevelt was told the news, she thought the couple was too young to marry (Eleanor was 19 and Franklin was 21). Sara then asked them to keep their engagement a secret for one year. Franklin and Eleanor agreed to do so. During this time, Eleanor was an active member of the Junior League, an organization for wealthy young ladies to do charitable work. Eleanor taught classes for the poor who lived in tenement houses and investigated the horrible working conditions many young women experienced. Her work with poor and needy families taught her a great deal about the hardships many Americans faced, leading to a life-long passion for trying to solve society’s ills. Historical/Getty Images Married Life With their year of secrecy behind them, Franklin and Eleanor publicly announced their engagement and then married on March 17, 1905. As a Christmas present that year, Sara Roosevelt decided to build adjoining townhouses for herself and Franklin’s family. Unfortunately, Eleanor left all the planning up to her mother-in-law and Franklin and thus was very unhappy with her new home. Plus, Sara would frequently stop by unannounced since she could easily enter by going through a sliding door that joined the two townhouses’ dining rooms. While being somewhat dominated by her mother-in-law, Eleanor spent between 1906 and 1916 having babies. In total, the couple had six children; however, the third, Franklin Jr., died in infancy. In the meantime, Franklin had entered politics. He had dreams of following his cousin Theodore Roosevelt’s path to the White House. In 1910, Franklin Roosevelt ran for and won a State Senate seat in New York. Just three years later, Franklin was appointed assistant secretary of the navy in 1913. Although Eleanor was disinterested in politics, her husband’s new positions moved her out of the adjoined townhouse and thus out of the shadow of her mother-in-law. With an increasingly busy social schedule due to Franklin’s new political responsibilities, Eleanor hired a personal secretary named Lucy Mercy to help her stay organized. Eleanor was shocked when, in 1918, she discovered that Franklin was having an affair with Lucy. Although Franklin swore he would end the affair, the discovery left Eleanor depressed and dejected for many years. Eleanor never truly forgave Franklin for his indiscretion and although their marriage continued, it was never the same. From that point forward, their marriage lacked intimacy and began to be more of a partnership. Polio and the White House In 1920, Franklin D. Roosevelt was chosen as the Democratic vice presidential nominee, running with James Cox. Although they lost the election, the experience had given Franklin a taste for politics at the top level of government and he continued to aim high- until 1921 when polio struck. Polio, a common disease in the early 20th century, could kill its victims or leave them permanently disabled. Franklin Roosevelt’s bout with polio left him without the use of his legs. Although Franklin’s mother Sara insisted that his disability was the end of his public life, Eleanor disagreed. It was the first time Eleanor had openly defied her mother-in-law and it was a turning point in her relationship with both Sara and Franklin. Instead, Eleanor Roosevelt took an active role in helping her husband, becoming his â€Å"eyes and ears† in politics and assisting with his attempts to recover. (Although he tried for seven years to regain the use of his legs, Franklin finally accepted that he would not walk again.) Franklin reentered the political spotlight in 1928 when he ran for governor of New York, a position he won. In 1932, he ran for president against incumbent Herbert Hoover. Public opinion of Hoover had been decimated by the 1929 stock market crash and the Great Depression that followed, leading to a presidential victory for Franklin in the 1932 election. In many ways, she had created an independent life for herself in New York and dreaded leaving it behind. Most especially, Eleanor was going to miss teaching at the Todhunter School, a finishing school for girls she had helped purchase in 1926. Becoming first lady took her away from such projects. Nevertheless, Eleanor saw in her new position the opportunity to benefit disadvantaged people nationwide and she seized it, transforming the role of the first lady in the process. Before Franklin Delano Roosevelt took office, the first lady generally played an ornamental role, mainly one of a gracious hostess. Eleanor, on the other hand, not only became a champion of many causes but continued to be an active participant in her husband’s political plans. Since Franklin could not walk and did not want the public to know it, Eleanor did much of the traveling he could not do. She would send back regular memos about the people she talked to and the sorts of help they needed as the Great Depression worsened. Eleanor also made many trips, speeches, and other acts to support disadvantaged groups, including women, racial minorities, the homeless, tenant farmers, and others. She hosted regular Sunday â€Å"egg scrambles,† in which she invited people from all walks of life to the White House for a scrambled-egg brunch and a talk about the problems they faced and what support they needed to overcome them. In 1936, Eleanor Roosevelt began writing a newspaper column called â€Å"My Day,† on the recommendation of her friend, newspaper reporter Lorena Hickok. Her columns touched on a wide range of often-controversial topics, including the rights of women and minorities and the creation of the United Nations. She wrote a column six days a week until 1962, missing only four days when her husband died in 1945. Reg Speller  / Getty Images The Country Goes to War Franklin Roosevelt won reelection in 1936 and again in 1940, becoming the first- and only- U.S. president to serve more than two terms. In 1940, Eleanor Roosevelt became the first woman ever to address a national presidential convention when she gave a speech to the Democratic National Convention on July 17, 1940. On December 7, 1941, Japanese bomber planes attacked the naval base at Pearl Harbor, Hawaii. Within the next few days, the U.S. declared war on Japan and Germany, officially bringing the U.S. into World War II. Franklin Roosevelt’s administration immediately began enlisting private companies to make tanks, guns, and other necessary equipment. In 1942, 80,000 U.S. troops were sent to Europe, the first of many waves of soldiers that would go overseas in the coming years. With so many men fighting the war, women were pulled out of their homes and into factories, where they made war materials, ranging from fighter planes and parachutes to canned food and bandages. Eleanor Roosevelt saw in this mobilization the opportunity to fight for the rights of working women. She argued that every American should have the right to employment if they wanted it. She also fought against racial discrimination in the workforce, the armed forces, and at home, arguing that African-Americans and other racial minorities should be given equal pay, equal work, and equal rights. Although she vehemently opposed putting Japanese-Americans in internment camps during the war, her husband’s administration did so anyway. During World War II, Eleanor also traveled all over the world, visiting soldiers stationed in Europe, the South Pacific, and other far-flung places. The Secret Service gave her the code name â€Å"Rover,† but the public called her â€Å"Everywhere Eleanor† because they never knew where she might turn up. She was also called â€Å"Public Energy Number One† due to her intense commitment to human rights and the war effort. First Lady of the World Franklin Roosevelt ran for and won a fourth term in office in 1944, but his remaining time in the White House was limited. On April 12, 1945, he passed away at his home in Warm Springs, Georgia. At the time of Franklin’s death, Eleanor announced she would withdraw from public life and when a reporter asked about her career, she said it had ended. However, when President Harry Truman asked Eleanor to become Americas first delegate to the United Nations in December 1945, she accepted. As an American and a woman, Eleanor Roosevelt felt that being the U.N. delegate was a huge responsibility. She spent her days before the U.N. meetings researching issues of world politics. She was particularly concerned with failing as a U.N. delegate, not only for herself but because her failure might reflect badly on all women. Rather than being seen as a failure, most regarded Eleanor’s work with the United Nations as a resounding success. Her crowning achievement was when the Universal Declaration of Human Rights, which she had helped draft, was ratified by 48 nations in 1948. Back in the United States, Eleanor Roosevelt continued to champion civil rights. She joined the board of the NAACP in 1945, and in 1959 she became a lecturer on politics and human rights at Brandeis University. Death and Legacy Eleanor Roosevelt was getting older but she didn’t slow down; if anything, she was busier than ever. While always making time for her friends and family, she also spent a lot of time traveling around the world for one important cause or another. She flew to India, Israel, Russia, Japan, Turkey, the Philippines, Switzerland, Poland, Thailand, and many other countries. Eleanor Roosevelt had become a goodwill ambassador around the world; a woman people respected, admired, and loved. She had truly become the â€Å"First Lady of the World,† as U.S. President Harry Truman once called her. And then one day her body told her she needed to slow down. After visiting a hospital and undergoing lots of tests, it was discovered in 1962 that Eleanor Roosevelt was suffering from aplastic anemia and tuberculosis. Some conditions that result in secondary narcolepsy include traumatic brain injury, tumors, and stroke. [1] A rare case of secondary narcolepsy was seen in a patient with self-inflicted genital injury. A 30 year old male was referred to Psychiatry from Surgery for a self-inflicted incised wound on hydrocoele. Since last 1 year, he had multiple episodes of 1. Sudden falls while working 2. Sleep during daytime often at unusual places 3. Periods of unresponsiveness during which he was aware but unable to move. During hospital stay, daytime somnolence, sleep paralysis and cataplexy were noted several times, but hallucinations were not consistently reported. Based on DSM-IV-TR Narcolepsy was diagnosed. Possible reasons for genital injury were 1. To remove fluid from swelling 2. Under sleep paralysis 3. Under Hypnogogic hallucinations. Patients EEG was normal. MRI brain showed Gliosis at cervico-medullary junction.MRI spine was advised to examine the cervico-vertebral junction but patient was lost to follow-up. But from history and investigations, it was concluded that he had secondary narcolep sy due to traumatic brain injury. Narcolepsy typically begins in the 2nd and 3rd decades of life and negatively impacts the quality of life of affected patients. Diagnosis relies on patient history and objective data gathered from polysomnography and multiple sleep latency testing. Treatment focuses on symptom relief through medication, education, and behavioural modification. It is an abnormality of the sleep mechanisms specifically, REM-inhibiting mechanisms and it has been studied in dogs, sheep, and humans. Narcolepsy can occur at any age, but it most frequently begins in adolescence or young adulthood, generally before the age of 30. The disorder either progresses slowly or reaches a plateau that is maintained throughout life.[2]. The prevalence of narcolepsy varies across countries and with different ethnic groups, and so the exact prevalence is not known. Prevalence estimates have been reported to be between 168 and 799 per 100,000 in most studies, although Japanese studies have indicated a higher prevalence of 1600 per 100,000.[2,3]. There are no genetic tests current ly available for clinical use to make a positive diagnosis of narcolepsy. Genetic testing may correlate best to narcolepsy when there is already clear cataplexy.[4]. Supporting the evidence for an environmental influence is the fact that the disease is not apparent at birth, but instead commonly has its onset during the second decade of life. Additionally, there are apparent precipitating factors such as head trauma, infection, and changes in sleep-waking habits that have been identified in some cases.[6] Chronic, daytime sleepiness is a major, disabling symptom for many patients with traumatic brain injury (TBI), but thus far, its aetiology is not well understood. Extensive loss of the hypothalamic neurons that produce the wake-promoting neuropeptide hypocretin (orexin) causes the severe sleepiness of narcolepsy, and partial loss of these cells may contribute to the sleepiness of Parkinson disease and other disorders. One study has found that the number of hypocretin neurons is significantly reduced in patients with severe TBI. This observation highlights the often overlooked hypothalamic injury in TBI and provides new insights into the causes o f chronic sleepiness in patients with TBI.[7] Amphetamine usage has been associated with addiction, psychosis and self-injurious behaviour. There are reports on two patients who severely and repeatedly mutilated their own genitalia while intoxicated on amphetamines and consider possible diagnostic aetiologies.[8] Genital mutilation is common in males compared to females.[9] But narcolepsy presenting as self inflicted genital injury has not been reported so far. That is why this is a rare case. Case History[D3]: A case of secondary narcolepsy was seen in a patient presenting as self-inflicted genital injury. A 30 year old Hindi speaking illiterate male was referred to Psychiatry from Surgery for a self-inflicted incised wound on hydrocele. After primary wound closure at surgical side, patient was taken transfer to Psychiatry for detailed assessment. When detailed history was obtained from patients elder brother and father, it was found that since last 1 year, he had multiple episodes of sudden falls while working at kitchen as he was a cook. He used to sleep during daytime often at unusual places like in the courtyard, once over the road and sometimes in bathroom. Patient also had periods of unresponsiveness during which he was aware but unable to move himself even on painful stimulation. During hospital stay, daytime somnolence, sleep paralysis and cataplexy were noted several times, but hallucinations were not consistently reported. Based on DSM-IV-TR Narcolepsy was diagn osed. But from history and investigations, it was concluded that he had secondary narcolepsy due to traumatic brain injury Discussion: Narcolepsy is a condition characterized by excessive sleepiness, as well as auxiliary symptoms that represent the intrusion of aspects of REM sleep into the wakingstate. The sleep attacks of narcolepsy represent episodes of irresistible sleepiness, leading to perhaps 10 to 20 minutes of sleep, after which the patient feels refreshed, at least briefly. They can occur at inappropriate times (e.g., while eating, talking, or driving and during sex). The REM sleep includes Hypnogogic and Hypnopompic hallucinations, cataplexy, and sleep paralysis. The appearance of REM sleep within 10 minutes of sleep onset (sleep-onset REM periods) is also considered evidence of narcolepsy. The disorder can be dangerous because it can lead to automobile and industrial accidents. Other symptoms include Hypnogogic or Hypnopompic hallucinations, which are vivid perceptual experiences, either auditory or visual, occurring at sleep onset or on awakening. Patients are often momentarily frightened, but within a minute or two they return to an entirely normal frame of mind and are aware that nothing was actually there. Here patient had symptoms of narcolepsy after head injury and patient himself injured his scrotum with sharp blade for which he had no clear memory and there was no history suggestive of epilepsy so diagnosis of secondary narcolepsy presenting as self-inflicted genital injury was considered.In this case, patient showed clinical features of narcolepsy as diagnosed by DSM- IV which was secondary type as there was history of multiple falls and MRI brain showed gliosis. But in this case, unusual presentation was genital self-inflicted injury. Possible reasons for genital injury are: Automatic behavior. Acting on Hypnogogic/Hypnopompic hallucinations. Due to impaired cognitive function/ judgment due to long-standing Narcolepsy. No cure exists for narcolepsy, but symptom management is possible. A regimen of forced naps at a regular time of day occasionally helps patients with narcolepsy and, in some cases, the regimen alone, without medication, can almost cure the condition. When medication is required, stimulants are most commonly used.[10] Although drug therapy is the treatment of choice, the overall therapeutic approach should include scheduled naps, lifestyle adjustment, psychological counselling, drug holidays to reduce tolerance, and careful monitoring of drug refills, general health, and cardiac status. TCZ does not only strive to maintain its leading position as Caribbean â€Å"N ° 1† State of the Art Furniture† Producer, but also to expand in novelty and quality that steers a higher level of customer satisfaction alongside profit maximization. This whole process has involved an integration of mission, objectives, implementation and evaluation; a proper time, cost, quality and human resources management: and a critical maintenance of all forms of motivations. The company is made up of 45 staff members. Briefly, it is headed by a CEO, with two Directors (Director of Production and Director of Budget) answerable to him. They direct the two sectors of the company (Production and Budget). The Budget sector has two managers (Purchase Manager and Sales Manager) answerable to the Director of Budget. Immediately under the Purchase Manager is the Accounting Supervisor, and directly under the Sales Manager is the Sales Supervisor. The Sales Supervisor oversees six employers while the Account Supervisor has five employees under his control. Meanwhile in the Production Sector only one manager (Manufacturing Manager) is answerable to the Director of Productions. Directly under the Manufacturing Manager are the Inventory Supervisor and Operations Supervisor. The Inventory Supervisor oversees 10 employees while the Operations Supervisor supervises 14 employees. This well structured organizational flow is mindful of effective communications, job duplications, conflict and other factors that can affect a team spirit and job dexterity. Dear investors as a result of this strategy, TCZ has grown substantially faster than the global â€Å"State Of The Arts Furniture† market. While the global market as a whole grew just 5%, ours increased by 13%. An important source of revenue, the delivery of unrivalled bed, mattress and electronics charges, increased 18%, although the market saw a 1% decrease. Our pro-forma operating margin improved to 28. 3%, an increase of half a percentage point. We took additional market share away from our direct competitors in the ‘State of Arts Furniture’ market, gaining one percentage point year-on-year, which takes us to a market share of approximately 21%. We thus sell approximately two and a half times more unrivaled electronic beds and, mattress than our nearest competitor, who had been the leading producer for 18 years, before we ever dominated the market Our main competitor is the ABC State of Arts Furniture Ltd. They are the longest in the market and have dominated the premier role for 18 years. They are known for quantity and timely delivery. Unlike our competitors we added other important ingredients such as quality, novelty and frequent customer satisfaction research. This accounted for why we have emerged as the leading company in the market for the past five years. Our few years of existence in the market would have attracted fear and false judgements from prospective investors against our maturity, stability and long-term success and survival. The Holt Case relates to the company’s snapshot, case situation and key management issues pertaining to the Holt Renfrew which was being operated in Canada. Company Snapshot Holt Renfrew was constituted as a hat and fur shop in Quebec City in 1837. The company is recognized as one of the elite high-end retailers of Canada. Almost 10 stores were operated by the company in seven cities of Canada in which cosmetics and designer fashions were being sold such as Oscar de la Renta, Gucci, Prada, Dolce & Gabbana and Armani. These products were usually imported from Asia, Europe and the United States. The top quality was the base of Holt Renfrew brand which included private-label and branded assortments and designers that were exclusively for men and women. The Holt Renfrew was then owned by Galen Weston who is a Canadian business leader heading The Wittington Group. Case Situation During the observation of case situation, it was revealed that the staff used to spend much of the time in telephonic communication for adjusting and confirming previous orders. The stock of merchandise was quite high due to which staff remains busy on phone lines for orders tracking, delivery status, confirmation and shipment with transportation service providers and suppliers. The goods were delivered to the distribution centre by suppliers without prior intimation which used to cause inconvenience in scheduling routine tasks. Even it was not possible to determine whether right quality and quantity is being received. Warehouses were so much loaded that only in DC; inventory was stored around $40 million worth which created a hindrance in tracking the shipment in a timely manner. Even the complaint was lodged by store managers regarding overstocks of merchandises which prove the worse condition of warehouses. Key Management Issues The key management issues could be the closure of secondary warehouse and the consolidation of operational warehouse into DC. It would be useful for DC if the addition of mezzanine floor up to 20,000 square feet with a cost of $1 million could be practiced. Additionally, the warehouse problems arose due to overloaded stock. The reason behind this fact was the less sales target being accomplished and improper system involving excessive merchandises which was ordered without prior requisition or sanction and the same was too seriously complained by the store managers.
